Nel 1989, Maria Rossi chiama la polizia per confessare di avere brutalmente ucciso tre persone durante un esorcismo a cui era stata sottoposta. Venti anni dopo, sua figlia Isabella cerca di far luce su quanto accaduto quella notte per capire se la madre sia pazza o realmente indemoniata e si reca a farle visita presso l'ospedale psichiatrico italiano in cui è rinchiusa. Assistendo a una serie di eventi inspiegabili, Isabella richiede l'aiuto di due giovani esorcisti che la introducono nel sottobosco degli esorcismi non autorizzati dal Vaticano. Con orrore, scopre così che la madre è posseduta da ben quattro demoni, simbolo di un male difficile da estirpare.
